Massasoit Community College now offers students the option to submit Paramedic documentation forms online, safely and securely, using the form below. Documents will no longer be accepted via email.

Tips for successful submission

• Please print legibly
• Personal signatures require blue or black ink.
• Make sure your Massasoit student ID is on each submitted page.
• Please upload your State of MA EMT certificate, National Registry EMT certificate and an image of your drivers license.
• Files must be in .pdf, .doc, .docx, or .jpg format.
